All returning players must register by Aug 23^rd or a $50 late fee will be assessed!!!

Registration Fees are as follows:
Mite:                  $400
Squirt C:           $650
Squirt B:           $800
PeeWee B:       $850
PeeWee A:       $950
Bantam:           $1000

Midget:             $800
U12:                  $650
U19:                  $775

**There is a $100 discount for multi-child families, and there is a 20% discount for first time members (mites excluded)

     Parent Code of Conduct
     Player Code of Conduct
     Consent to Treat
     Waiver of Liability
     Work Program Agreement
Include two separate payments - one for registration and $150 work program fee (per family) Mail to by 8/23/08:  Summit Hockey, c/o Jennifer Hernandez, 32 Sage View Court, Dillon, CO 80435 and it needs to be received no later than August 23, 2008.

To complete registration everyone must register with USA Hockey online and attach bar coded registration form. Click here for USA Hockey Online Registration.

Please download and print the Summit Hockey Registration packet. All forms must be completly filled out to complete registration.

JERSEYS will be available to purchase - $140.00 per set.  No used jerseys are available at this time, but that could change on registration day.
 
SOCKS will be distributed to all who need new socks for the season.
 
WARM-UPS will also be available to order.
